Experimental Studies on Hematologic Findings, and Histopathological Changes of Liver and Spleen after Side-to-Side, End-to-Side and H-type Portacaval Shunts


Abstract
Side-to-Side, End-to-Side and H-type Portacaval Shunts

Sid-to-side, end-to-side and H-type portacaval shunts were performed on 15 mongrel adult dogs, and the auther observed changes of hematologic values, liver function, anastomotic stomal patency, and histopathological changes of liver, spleen and anastomotic sites after shunt operation.
The results are as follows:
1. In hematologic findings, the postoperative mean values of RBC, Hb and Ht were decreased in sideto-side, and end-to-side shunt groups.
2. In side-to-side shunt groups, postoperative mean values of RBC, WBC, Hb, Ht and platelet were
decreased. Auther noted that there were statistically significant decrease in blood cells at postoper-
ative 3rd week and statistically significant decrease of WBC through-out the 6weeks.
3. `In liver function test, serum total protein, globulin and GOT were increased in all shunt groups g but¢¥more marked increase was seen in side-to-side and H-type portacaval shcnt groups. Serum 4 ..cholesterol was decreased in all shunt groups.
4.. , As histopathological changes of liver, hepatic cell degeneration, disarray -in cell cord, . venous congestion, : portal fibrosis and. protal infiltration were observed, whereas there were seen sinusoid: congestion, hemorrhage, atrophy of white pulp, hemosiderin pigmentation as microscopic abnormalities of spleen.
In side-to-side and H-type portacaval shunt groups, more maked abnormalities were observed in comparison with the end-to-side portacaval shunt group. In anastomotic sites, hemorrhage, fibrosis and foreign body reaction were seen and histopathological changes of similar degree were observed in all shunt groups.
